Classes meet the first Wednesday of every month from 11am - 12pm and the third Thursday of each month from 6:30pm - 7:30pm at the OHMS Community Education Health Center (128 SW I St. Grants Pass).

*Entrance is on H St. Alley in the Free 3 hour parking lot.*

About Pathways to Care

Pathways to Care (PCN) is a registered 501 (c)3 Oregon non-profit corporation and a collaborative effort between service providers and community members dedicated to connecting people in our community with the information and services that they need.

Founded in 1997 as a collaboration between medical service providers to aid the under and uninsured of Josephine County overcoming barriers preventing access to health care, PCN has been deeply involved in the community through the Heart to Heart Project, EMS Week, Wee One’s, and through the Wellness on Wheels (W.O.W.).

Currently PCN is looking to expand our role in the community and the role of our organization to provide needed information and resources while collaborating to provide needed services to all the members of our community.
